Notice of Privacy Practices This notice describes how health information about you may be used and disclosed and how you can get access to this information.
PLEASE REVIEW THIS INFORMATION CAREFULLY. The privacy of your health information is important.
LEGAL DUTY We are required by federal and state law to maintain the privacy of your health information and to notify affected individuals following a breach of their unsecured protected health information. We are also required to give you this Notice about our privacy practices, our legal duties and your rights concerning your health information. We must follow the privacy practices that are described in this Notice while it is in effect. The requirement of this notice took effect on 04/14/2003 and will remain in effect until we replace it. You may request a copy of our Privacy Practice Notice at any time. If and when permitted by applicable, we have the right to change our privacy practices; if we do so, we will notify you in writing of these changes.
USE AND DISCLOSURES OF HEALTH INFORMATION We are permitted, by law, to use and disclose health information about you for reasons concerning treatment, payment and healthcare operations. If other law, including state law, prohibits or materially limits the ability to make a use or disclosure that would otherwise be permitted under HIPAA, Coastline will only make the uses and disclosures permitted under the more stringent law.
Examples:
Treatment: We may disclose your health information to a physician or other healthcare provider that is providing treatment or other health services to you.
Payment: We may use and disclose your health information to obtain payment for services that we provide you.
Operations: We may use and disclose your health information in connection with our healthcare operations, which include administration and planning and other tasks that help us improve that quality.
Family and Friends: We may disclose your health information to a family member, relative or a friend that has been identified by you while you are present. If you are not present, professional judgment will be utilized to determine whether a disclosure is required or in your best interest. We will only disclose information that is believed to be relevant to the person’s involvement with your health care or payment related to your health care. We may also disclose your health information in order to notify such persons of your location, general condition or death.
Requirements of the Law: We may use or disclose your health information when we are required to do so by Law.
Victim of Abuse or Neglect: We may disclose your health information to authorities if reasonable belief is that you are a possible victim of abuse, neglect or domestic violence. We may disclose information to the extent necessary to avert an additional serious threat to your health or safety or the health or safety of others.
Public Health Activities: We may disclose your health information to public health authorities for the purpose of preventing or controlling disease or preventing injury; to alert a person who may have been exposed to a communicable disease; to report information about products and services under the jurisdiction of the U.S. Food and Drug Administration; to report information to a health oversight agency that is responsible for ensuring compliance with governmental rules and regulations, such as Medicare and Medicaid.
National Security: We may disclose to military authorities the health information of Armed Forces personnel under certain circumstances. We may disclose to authorized federal officials health information required for lawful intelligence, counter intelligence and other national security activities.
Appointment, Refill, and Prescription Reminders and Communications: We may contact you to provide you with information about a drug currently prescribed, appointment reminders, and refill reminders, including essential information such as dates, times, locations and the name of any relevant company/provider.
Worker’s Compensation: We may use or disclose your health information to the extent necessary to comply with state laws relating to workers’ compensation.
Disclosures Requiring your Authorization: For any reasons other than those listed in this notice, we may only use or disclose your health information with your written authorization. Your authorization must also be obtained prior to using your health information for any marketing activity.
Your Rights To Your Personal Health Information
Access to Record: You may have access to your health information, with limited exceptions. Request must be made in writing, utilizing our Record Access Request Form. We may charge a reasonable fee to compensate for time and materials.
Revocation of your Authorization: You may revoke your authorization to disclose your health information at any time. Request must be made in writing, using our Authorization Request Form.
Restriction of Information: You may request that we place restrictions on our use or disclosure of your health information. You must make your request in writing by sending a letter that specifies the type of information to be restricted and to whom the information is to be restricted from. Requests should be sent directly to the address listed in the heading of this Notice. We will consider all requests; however, we are not required to agree to the request. We will respond to all such requests in writing.
Disclosure Accounting: You may request a list of instances in which we (or our business associates) disclosed your health information for purposes, other than treatment, payment, Healthcare operations and certain other activities, for the last 6 years, but not before April 14, 2003. You must make your request in writing by sending us a letter that specifies the type of information and the time period involved. Requests should be sent directly to the address listed in the heading of this Notice. If you request this information more than once in a 12-month period, we may charge you a reasonable fee to compensate for our time and materials.
Alternative Communication: You may request that we communicate with you about your health information by alternative means or to an alternative location. You must make your request in writing by sending a letter that specifies the alternative means or location and provide satisfactory explanation how payments will be handled under the alternative means or location you have requested. Requests should be sent directly to the address listed in the header of this Notice.
Amendment: You have the right to request that we amend your health information. You must make your request in writing by sending us a letter that explains why the information should be amended. Requests should be sent directly to the address listed in the header of this Notice.
We will comply to your request unless we believe that the information to be amended is accurate and complete.

Patient Bill of Rights and Responsibilities
To ensure the finest care possible, as a patient receiving our pharmacy services, you should understand your rights and responsibilities as well as your involvement in your own plan of care.
To receive information about product selection, including suggestions of methods to obtain medications not available at the pharmacy where the product was ordered.
To request and receive complete and up-to-date information relative to your condition, treatment, alternative treatments, risk of treatment or care plans.
To receive information on how to access support from consumer advocates groups.
To receive information to assist in interactions with the organization.
To receive information about health plan transfers to a different facility or Pharmacy Benefit Management organization that includes how a prescription is transferred from one pharmacy service to another.
To Receive pharmacy health and safety information to include consumers rights and responsibilities.
To request prescription readers if you are an Oregon resident.

The right to identify the program’s staff members, including their job title, and to speak with a staff member’s supervisor if requested.

As our patient, you have the Responsibility to:
To notify your Physician and the Pharmacy of any potential side effects and/or complications.
To submit any forms to the pharmacy that are necessary to provide appropriate service to you.
To notify Coastline Pharmacy of any demographic changes including address, insurance, allergy or prescriber information.
Co-Pay Assistance and Payment
Prior to dispensing your order, a staff member will reach out to you regarding any financial obligations you may incur.
These obligations include but are not limited to: out-of- pocket costs such as deductibles, co-pays, coinsurance, annual and lifetime co-insurance limits and changes that occur during your enrollment period.
This co-payment is due at the time of shipping or pickup. We accept Visa®, MasterCard®, American Express®, and Discover®. We can maintain your credit card information on file in a secured environment.
We can assist you in helping with copayments and financial barriers in helping to start your medication.
Insurance Claims
We will submit claims to your health insurance carrier on the date your prescription is filled. If the claim is rejected, a staff member will notify you, as necessary, so that we can work together to resolve the issue. There may be financial obligations if your health benefit plan is an out of network pharmacy, if that happens the Organization will provide notice of this in writing.
Refills
You will be contacted by a team member up to 14 days prior to your refill date. If you would like to contact us for a refill, you can call us and speak to a customer service representative, technician or pharmacist to process your refill requests. If needed, a pharmacist will assist you with a process to refill a prescription which would otherwise be limited by your prescription benefit plan.
Prescription Transfers
If our pharmacy is unable to meet your needs, we can transfer your prescription to the appropriate pharmacy of your choice. Please call us.
Adverse Drug Reactions

Drug Recalls
If your medication is recalled, the pharmacy will contact you, with further instructions, as directed by the FDA or drug manufacturer.
Drug Substitution Protocols
Our pharmacy will always use the most cost-efficient option for you. From time to time it is necessary to substitute generic drugs for brand name drugs. This could occur due to your insurance company preferring the generic be dispensed or to reduce your copay. When available, our pharmacy will default to generic to save you money. We will use brand name medication at your or your prescriber’s request.
Proper Disposal of Sharps
Place all needles, syringes, and other sharp objects into a sharps container. This may be purchased at the pharmacy.
Proper Disposal of Unused Medications
For instructions on how to properly dispose of unused medications, check with your local waste collection service. Do not flush unused medications or pour down a sink or drain. You can also check the following websites for additional information:

We want you to be completely satisfied with the care we provide, however, under certain circumstances chemical degradation of drugs may occur. If you or your caregiver have any questions or issues regarding items such as order delays, order status, errors or need assistance accessing evidence-based health related information such as common conditions, diagnosis, treatment diagnostics and interventions please contact us directly at 866-758-1957 and speak to one of our staff members.

Our Emergency and Disaster Preparedness Plan
Coastline Pharmacy has a comprehensive emergency preparedness plan in the event a local disaster occurs. Our primary goal is to continue to service your prescription care needs.
The pharmacy will call you 3-5 days before an anticipated local weather disaster emergency utilizing the weather updates as point of reference.
If you are not in the pharmacy local area but reside in a location that will experience a weather disaster you are responsible for calling the pharmacy 3-5 days before the occurrence.
The pharmacy will send your medication via FedEx or USPS next day delivery during any suspected inclement weather emergencies.
If the pharmacy cannot get your medication to you before an inclement weather emergency occurs the pharmacy will transfer your medication to a local pharmacy, so you do not go without medication.
If a local disaster occurs and the pharmacy cannot reach you or you cannot reach the pharmacy, please listen to your local news and rescue centers for advice on obtaining medication. Visit your local hospital immediately if you will miss a dose.
The pharmacy recommends all patients leave a secondary emergency number.
Emergency and Disaster Planning Information
Have an emergency plan in place with your family and caregiver and make sure to include all your health and medical needs including healthcare-related legal documents and a list of current Medications, contact information for family, physicians, hospital, etc., preparation of equipment and supplies for at least one week, evacuation plans, and care for pets as applicable.
If you live outside of the local pharmacy area, and there is a disaster in your area, please contact our pharmacy to instruct us on how to deliver your medication. This will ensure your therapy is not interrupted.
If you have an emergency that is not environmental but personal and you need your medication, please contact the pharmacy at your convenience and we will aide you.

Return Policy
Our return policy has been established to protect our customers. For your safety, Coastline Pharmacy cannot accept returns of properly filled medication orders. Products returned outside of this return policy or without authorization may be discarded without refund.
